It was first thought that the tritable acids of quinic, malic and citric acids were the mechanism that treated and prevented UTIs. In 1998, Ahuaji, S., and others reported that "the cranberry irreversably inhibits the expression of P-fimbriae of E. coli. Ahujai was able to observe the E. coli bacteria actually change its cell structure, disabling the bacteria's fimbriae and inhibited agglutination (colonization)" by simply adding a small amount of cranberry juice to a petri dish containing E. coli.

The National Institute of Health reports that "in the US annually over nine million (9,000,000) doctor office visits and over one million (1,000,000) hospitalizations occur involving UTIs." This figure does not account for the millions of cases that are self treated and, thus, unreported.

While centuries of folk wisdom have touted certain fruits and vegetables as having anti-microbial properties, placebo-controlled clinical trials to test the effects of these foods and their 'phytochemicals' are sorely lacking. One exception to this is research on cranberry juice cocktail and UTI conducted by scientists at the prestigious Harvard Medical School in 1994 (Avorn et al).

In the Harvard study, 153 elderly women drank ten ounces (300 ml) of cranberry juice cocktail (27% cranberry juice sweetened with saccharin) each day for six months. UTI's were found in 28% of the women who received the placebo juice while those who drank cranberry juice cocktail had a UTI rate of only 15%. Additionally, these positive effects appeared after only four to eight weeks into the study (see chart from Avorn et al.).
